This case study forms part of the What Good Looks Like report on people with a learning disability and autistic people. This co-produced report was commissioned from the Building the Right Support (BTRS) Advisory Group, as part of the wider action plan developed by the Building the Right Support Delivery Board. It has been supported by Partners in Care and Health.

With the seven-day working strategy, Hertfordshire aimed to improve the flow from the acute into the community, ensuring discharges are not delayed over the weekend while service users wait for a package of care due to processes outside of the Monday to Friday norm. A new integrated adult Community Learning Disability (LD) Team has been uniquely co-located with children’s social care and education services in purpose-designed premises to improve outcomes for residents of all ages with disabilities.

Hampshire County Council has made assistive technology a mainstream part of its adult social care work. The approach has helped it achieve substantial savings with 100 new referrals being made each week into the service.
