Around 14,000 NHS Health Checks take place in Newham each year. Newham Council commissions the borough’s GP federation to deliver them, which sub-contracts to GP practices. Along with some neighbouring boroughs, Newham benefits from excellent data through a partnership with the Clinical Effectiveness Group based at Queen Mary University of London.

Norfolk County Council have turned their NHS Health Check programme performance around, going from one of the lowest-ranked councils in the East of England region, in terms of percentage of the eligible population receiving a Health Check, to one of the best in England.

West Sussex County Council works with a range of providers to deliver the NHS Health Check programme across the county. This includes offering checks in community settings such as libraries, village halls, workplaces and high-street hubs, as well as in clinical settings.

NHS Newcastle Gateshead clinical commissioning group (CCG), was selected to be part of NHS England New Care Models Care Home Vanguard Programme. One of the initiatives to be introduced was the ‘transfer of care bag’, which sought to improve communication between hospital and care home teams when residents moved between both settings. This example of a local initiative forms part of our managing transfers of care resource.

Like many areas, Norfolk has seen rising numbers of people coming to their sexual health clinics for testing. To help ease the pressure, an online service for the four core STIs has been developed. It has proved remarkably successful. This case study forms part of our sexual health resource.

Norfolk Health and Wellbeing Board (HWB) is the forum for system leaders across the wider local health and care system and works closely with the Norfolk and Waveney Sustainability and Transformation Partnership (STP) (Waveney is an area in East Suffolk). Norfolk has a well-established Healthy Libraries scheme that is having a significant impact on social isolation and loneliness. The library service runs a range of initiatives from singing and colouring groups to a reading project aimed at people who are housebound. They have played a vital role in keeping people living in the large rural county connected both before and during the pandemic.

At more than 5,000sq kilometres, Norfolk is one of the largest counties in England. To cover such a big area, the county council has set up a network of mobile testing teams rather than rely on static sites. It has allowed the teams to target testing at the neighbourhoods with the most need as well as support local businesses and big events.
