Medway: Home First

Home First is an initiative developed through the partnership of Medway Foundation Trust Hospital, Medway CCG, Medway Council and Medway Community Healthcare. It provides support for patients medically fit to be discharged, but who still require additional home support. This example of a local initiative forms part of our managing transfers of care resource.

Melton Borough Council: online and hybrid meetings

There had been intermittent problems with the audio system in the council chamber for some years before the pandemic lockdown which had caused difficulties in public meetings and generated complaints from members of the public. This feedback was taken extremely seriously and resulted in a commitment from councillors and officers to review the current provision because it was no longer deemed fit for purpose.

Finding it all at the YMCA Mendip District Council's family mediation service is provided in partnership with the YMCA. It is funded through the council's housing budget and helps young people at risk of homelessness return to the family home.
