NHS Greater Manchester Integrated Care Partnership and the Greater Manchester Older People’s Network: Reducing self-harm in older people

Research published in The Lancet in 2018 (Morgan et al) found that there is an elevated risk of suicide in older people who self-harm, yet older people do not always get access to specialist care. This case study was done jointly with the National Suicide Prevention Alliance and forms part of our suicide prevention resource.

NHS Health Checks: London Borough of Newham

Around 14,000 NHS Health Checks take place in Newham each year. Newham Council commissions the borough’s GP federation to deliver them, which sub-contracts to GP practices. Along with some neighbouring boroughs, Newham benefits from excellent data through a partnership with the Clinical Effectiveness Group based at Queen Mary University of London.

NHS Newcastle Gateshead CCG: Bringing care homes from the periphery in to the system

NHS Newcastle Gateshead clinical commissioning group (CCG), was selected to be part of NHS England New Care Models Care Home Vanguard Programme. One of the initiatives to be introduced was the ‘transfer of care bag’, which sought to improve communication between hospital and care home teams when residents moved between both settings. This example of a local initiative forms part of our managing transfers of care resource.
