Leeds City Council has a clear focus on the social care workforce, its aim is to support the social care workforce to have the best skills, founded upon high quality research and evidence that improves the health and wellbeing of the people of Leeds. In order to deliver an effective, skilled social care workforce in Leeds, the DASS has invested in the organisational and workforce development team to deliver a range of services.

Wirral has a large number of care homes and had a significant number of non-elective admissions from care home residents. It was a priority to ensure that care was provided in the patient’s place of residence wherever possible, as a non-elective admission can have a detrimental effect on frail older people. This example of a local initiative forms part of our managing transfers of care resource.

Health and care leaders worked with the voluntary and community sector by investing £20,000 to support the sector to build its information sharing capability to achieve compliance and facilitate further information sharing and social prescribing. This example of how local areas are working to implement overall system change forms part of our care and health improvement digital and information resource.

This collaborative project set out to help people with learning disabilities live more independently, particularly after the Covid pandemic. This was done by developing and implementing virtual support through a provider that guides people through information and learning materials to develop their employability and better manage their mental health and wellbeing.

Derbyshire County Council has been working with local partners to develop and deliver a clear plan for use of the improved Better Care Fund. The approach is helping to maintain capacity across both social work and the independent sector, and supporting system flow and hospital discharges. ‘The Bridge’ is a tool for data visualisation, designed to display public sector data and provide effective economic forecasting and market insights. Follow-on funding was granted to share the project and its learning with other councils.

This case study forms part of the What Good Looks Like report on people with a learning disability and autistic people. This co-produced report was commissioned from the Building the Right Support Advisory Group, as part of the wider action plan developed by the Building the Right Support Delivery Board. It has been supported by Partners in Care and Health.

The development of a transitions service offered by Hammersmith & Fulham Council demonstrates the good practice set out in several key domains of this outcomes and improvement framework.

There is strong partnership working in Greenwich to develop and improve the local offer. The overall objective is that people at the end of their lives and their families can be well supported and their outcomes improved through care and support that is integrated around the individual. United Pride Friends is a group service for members of the LGBTQ+ community with Autism and Learning difficulties. The primary purpose of their group is giving members a safe place to meet new people and make friends.
