Local area approaches – working to improve things

Local practice and approaches focused on reducing the use of specialist hospital beds for people with a learning disability and/or autism and enabling people to live good lives in the community.


By sharing information and learning in this way, area partners can work together and learn from each other to improve outcomes for people. The following areas shared their approaches with us.

Transforming Care Partnership (TCP) approaches to engaging with the care and support market

The 'So what, what next?' project: supporting people with a learning disability, autism or both to use their skills and interests to play a part in the community

The So What, What Next? project was designed by the Transforming Care empowerment steering group to look at ways of supporting people with a learning disability or autism who have recently been discharged from hospital to explore their skills and passions and to find ways to contribute these to their local communities. The focus was on supporting people to use their strengths, become active citizens and to grow their independence. Case studies were also previously collected and shared as part of the work of the Winterbourne Joint Improvement Programme which completed its programme of work on 31 March 2015 to be replaced with the Transforming Care programme.
